This is a prank that uses a piercing gun (piercer) that can easily make a piercing hole.
You say you’re going to pierce their ear with the piercer, but without actually touching their ear, you put on a magnetic earring instead.
Then you say, “I messed up!” and smear fake blood all over your hand, touch their ear, and make it look like their ear is bleeding.
Since it’s their own ear, even if they look in the mirror, they can’t see it very clearly, right? You might wonder whether they’d fall for it when there’s no real pain or sensation, but seeing blood would probably give them a scare.
